Lake County Chemical Dependency
Program
Information and Client Rights
Lake County Chemical Dependency Program is certified by the
State of Montana Department of Public Health and Human
Services to provide prevention, education, assessment,
treatment and aftercare to residents of Lake County.
This certification is renewed by annual evaluation and is an
assurance that our services meet standards set by state law.
If we believe we cannot provide appropriate service to you, we
will refer you to a suitable provider.
Client Rights Statement:
1. Every client has the right to be treated without regard to
physical or mental disability, religious preference, race,
creed or color, unless such conditions make treatment at LCCDP
non-beneficial or hazardous.
2. Every client has the right to have all clinical and
personal information treated confidentially in communications
with individuals not directly associated with Lake County
Chemical Dependency Program. Release of information of a
clinical or personal nature can only be done with the express
written approval of the client. The one exception to this
policy is when not releasing such information would be in
direct violation of state and/or federal law. Information will
be released to LCCDP staff on a need-to-know basis.
3. Every client has the right to respectful and considerate
care.
4. Every client shall be fully informed, prior to or at the
time of admissions and during his/her stay at our facility, of
the rights and responsibilities set forth in this statement of
Client Rights and all rules governing client conduct and
responsibilities.
Problem Resolution Procedures:
Lake County Chemical Dependency Program believes each client
is entitled to have the opportunity to resolve non-related
non-emergency problems promptly and fairly. The following
procedures are available for problem resolution.
Administration may, upon receipt of the form, request the
client fill out an incident report form and conduct a personal
interview. Forms are available from the administrative staff.
1. The primary counselor assigned each client is the initial
contact person for problem resolution. Prior to other
measures, a counseling session with the primary counselor
should be used for communication of problems.
2. The Program Director is available to meet with any client
who maybe experiencing problems which cannot be resolved by
your primary counselor. The Director may be contacted through
the clerical staff.
3. When a care-related problem is not resolved through the
primary counselor, or the Director, the client should use the
written form to communicate. Administration may, upon receipt
of the form, request the client fill out an incident report
form and conduct a personal interview. The written report
received by administration is forwarded to the Chairperson of
the Board of Directors, who may in turn review the issue with
the client.
4. The exceptions to these procedures are emergency events or
crisis situations in which clients should immediately fill out
incident report forms obtained from clerical staff, and which
should be promptly returned for immediate attention.
